• The forms must be completed by all Full-Time undergraduate students including those awaiting graduation.
• To print the form, you will need to enter your matric no. and scan passport photograph into the form.
• The filled asterisks on the forms are compulsory.
• If parent, guardian/sponsors are same, still enter guardian and sponsor.
• If more than once(receipts), enter relevant information.
• After completing all relevant information, click print student verification certificate.
